Awards

Each year at the Annual Meeting, AAHCM presents the following prestigious awards: House Call Provider awards, the Dan Gilden Creative Investigator award, and the Lifetime Achievement award. In 2022 we replaced the Lifetime Achievement award with the Bob Sowislo Person of the Year Award; in 2023 the Academy added the House Call Interdisciplinary Team of the Year Award to highlight the changing focus in the field of home-based medicine from individual work to a team approach.


 

House Call Physician of the Year

 

The House Call Physician of the Year recognizes a home care physician who provides the highest quality of services and innovative programs and who demonstrates excellent and continued dedication to the field of home care medicine. (Must be an AAHCM member)

House Call Educator of the Year

 

The House Call Educator of the Year recognizes an educator who mirrors the highest standards of excellence in teaching home care medicine, has made significant educational impacts on the academic advancement of students studying home care medicine, and has made long-standing contributions to the profession in the academic setting. The recipient must be an AAHCM member.

House Call Clinician of the Year

 

The House Call Clinician of the Year recognizes an outstanding clinician who has made significant contributions to the delivery of quality healthcare to homebound patients, and demonstrates the important value of clinicians in the field of home care medicine. A clinician may be defined to include nurse practitioners, social workers, and physician’s assistants. The recipient must be an AAHCM member.

Dan Gilden Creative Investigator Award

 

The Dan Gilden Creative Investigator Award is given to an exceptional investigator for their innovative body of work advancing the field of home care medicine. This award recognizes an investigator who has made great contributions to the field of home care medicine. Nominees and nominators should have experience in the field of home care medicine, but do not need to be current AAHCM members.

Bob Sowislo Person of the Year Award

Formerly named the Lifetime Achievement Award, this award will be presented annually to person who embodies qualities that Bob showed every day: Humility, Persistence and Dedication to Human Potential. Nominees need to be current AAHCM members in good standing, with a minimum of 10 years experience in the field of home care medicine.

House Call Interdisciplinary Team of the Year

AAHCM delivers on the promise of interdisciplinary, high-value health care in the home for all people in need by promoting the art, science, and practice of home care medicine. The AAHCM House Call Interdisciplinary Team of the Year Award recognizes an outstanding interdisciplinary team that has made significant contributions to the delivery of quality healthcare to homebound patients, and demonstrates the important value of a team in the field of home care medicine. A team must be made up of at least three providers (i.e. NP, PA, physician, social worker, physical and occupational therapist, pharmacist), at least one member of the team must be an AAHCM member.
