Get to Know Kristofer L. Smith, MD, MPPConsultant As Chief Medical Officer of Optum at Home, Dr. Smith oversaw clinical and care management programs for over 1.5 million patients, including 1.1M DSNP and 0.5M MA lives. He was responsible for HEDIS/STAR, clinical documentation, quality and affordability initiatives. He oversaw clinical model design, quality of care standards and clinical improvement activities. He worked closely with provider groups and plan partners to identify collaboration opportunities. Prior roles include Chief Medical Officer of Landmark Health, Chief Clinical Officer at Prospero Health, President of naviHealth’s Home-based Medical Care division and SVP of Population Health at Northwell Health as well as CMO of Northwell’s commercial insurance plan. At Northwell, his team built nationally leading home-based medical care, bundled payment, Medicaid care coordination, and readmissions reduction programs. He has published papers on advanced care models for the frail elderly and regularly gives national talks on health policy, the frail elderly and the intersection of payment reform and clinical redesign. He currently serves on the board of the American Academy of Home Care Medicine. He worked as a house calls physician for more than a decade, providing primary and palliative care to the frail homebound elderly.
