Get to Know Thomas Lally

Dr. Lally is a dedicated house call physician and the CEO of Bloom Healthcare, an employee-owned primary care practice that specializes in providing senior care in the home. Founded by Dr. Lally in 2003 as Physician House Calls, Bloom Healthcare has been at the forefront of delivering coordinated and exemplary care to a geriatric population. With a strong passion for innovation, Dr. Lally has been instrumental in developing new care delivery models and identifying treatment opportunities for the complex, homebound patient population.

Dr. Lally is a national leader in the treatment of dementia and has made significant contributions to clinical education, guiding providers and raising awareness in the broader community about dementia care. He served as the President and Chief Medical Officer for Kindred Healthcare’s Home-based Primary Care division from 2016 to 2018, where he implemented a best-in-class clinical delivery model across multiple states. Before his role at Kindred, Dr. Lally founded and led Physician House Calls, which became one of the largest and most sophisticated home-based primary care models in the United States.

In addition to his work in primary care, Dr. Lally co-founded Honor Hospice in 2013, a mission-driven hospice provider that worked closely with Physician House Calls to provide integrated care. He was named Physician of the Year in 2014 by the American Academy of Home Care Medicine for his contributions to the field. Outside of his professional work, Dr. Lally enjoys spending time with his family, coaching his daughters’ ski racing teams, and engaging in outdoor activities such as climbing.
