Shawna L. Ramey, CPC CPMA, postacute medicine consultant, Health Dimensions Group
Ms. Ramey has more than 20 years of primary care and geriatrics practice management experience, including as a practice manager for a diverse geriatrics medical practice of more than 20 medical providers at Summa Health System. She also has clinical experience in house call programs, palliative medicine, behavioral health, and skilled nursing facility practices. As a certified coder and auditor, Ms. Ramey provides chart coding audits and compliance reviews across healthcare settings as well as provider and staff education on coding documentation requirements and new code and payment opportunities. Her expertise also includes operational and revenue cycle assessments in venues such as geriatric assessment clinics, house call programs, and programs of all-inclusive care for elders.

Ms. Carlson advises acute and postacute organizations on strategic design and execution of integrated care delivery and value-based payment models, including bundled payment, preferred network development, care coordination, and community medicine, linking patient-focused care with emerging reimbursement and risk-bearing opportunities with a focus on care transformation. Dr. Carlson brings more than 30 years of healthcare experience throughout the care continuum, including development and administration of postacute services and communities such as continuing care campuses, skilled nursing facilities, home- and community-based services, and housing for seniors.
Gary Swartz, JD MPA, associate executive director, American Academy of Home Care Medicine
Mr. Swartz has more than 30 years of experience in medical practice management and health systems management, consulting and recruitment. His background includes work in health policy and advocacy and audit and compliance for health systems. In his role with AAHCM, he provides practice management and regulatory advice to medical practices and organizations across the country. Mr. Swartz also provides public policy service for the Academy and leads a best practices and evidence gathering learning collaborative for practices participating in Independence at Home, a Medicare shared savings demonstration.
