The House Call Project (Part of the Home Centered Care Institute) and Tom Cornwell, MD, AAHCM Past President and CEO of the Home Centered Care Institute have again generously offered to match the donations of the Board and membership up to $30,000.

Our promise of interdisciplinary, high value health care in the home for all people in need often enables our patients and their families to continue to enjoy long-standing traditions and create new ones. Traditions are the ties that bind.  Your ongoing gift will help to ensure that the work of the Academy will continue on into the future. 

From IAH and far-reaching public policy issues to planning the newly free-standing Annual Conference, and executing the priority goals of the strategic plan, the Academy has a robust and ambitious agenda. This year, we have been able to establish a collaboration with The House Call Project to develop education and a sustainable workforce.  We are also implementing a grant from The John A. Hartford Foundation entitled “Building Awareness and Engaging Payers to Bring Home-Based Primary Care into the Mainstream” to help move the field into the mainstream of U. S. health service delivery, and improve the care and outcomes for vulnerable older adults.

In the New Year, we will push ourselves higher and further than ever before because the practice of home care medicine demands it, our frail, elderly and homebound patients deserve it. Please make a generous gift today to keep the work of the Academy and the tradition of our mission alive. The matched funds will be earmarked as sponsorship for the 2017 Annual Meeting.
