Our promise of interdisciplinary, high value health care in the home for all people in need often enables our patients and their families to continue to enjoy long-standing traditions and create new ones. From IAH and far-reaching public policy issues to planning the newly free-standing Annual Conference, and executing the priority goals of the strategic plan, the Academy has a robust and ambitious agenda. This year, we have been able to establish a collaboration with The House Call Project to develop education and a sustainable workforce. We are also implementing a grant from The John A. Hartford Foundation entitled “Building Awareness and Engaging Payers to Bring Home-Based Primary Care into the Mainstream” to help move the field into the mainstream of U. S. health service delivery, and improve the care and outcomes for vulnerable older adults.
In the New Year, we will push ourselves higher and further than ever before because the practice of home care medicine demands it, our frail, elderly and homebound patients deserve it.
